What Does Medicare Not Cover?

When you first sign up for Medicare, it’s easy to assume it covers everything — after all, it’s government health insurance, right? But here’s the reality: Medicare covers a lot, but it doesn’t cover everything.


Knowing what’s not covered can help you plan ahead and avoid surprise bills. Let’s walk through the big gaps.


1. Long-Term Care (Custodial Care)


Medicare will cover short-term stays in a skilled nursing facility after a hospital stay, but it does not cover long-term custodial care — like help with bathing, dressing, or eating if you need full-time care at home or in a nursing home.


This is one of the most common misunderstandings, and long-term care costs can add up fast. That’s why some people consider long-term care insurance or set aside savings for this need.


2. Most Dental Care


Original Medicare doesn’t cover routine dental checkups, cleanings, fillings, or dentures. Some Medicare Advantage plans offer dental coverage, but you’ll want to check your plan’s details.


3. Vision and Hearing


Routine eye exams, eyeglasses, and hearing aids are not covered under Original Medicare. Again, some Medicare Advantage plans include vision and hearing benefits, so this is something to look for if those services matter to you.


4. Prescription Drugs (Without a Part D Plan)


Medicare Parts A and B don’t include prescription drug coverage. You need a separate Part D plan or a Medicare Advantage plan that bundles in drug coverage to help cover medications.


5. Medical Care Outside the U.S.


If you’re traveling abroad, Medicare generally won’t cover your healthcare costs. Some Medigap plans offer limited foreign travel emergency coverage, or you can buy separate travel insurance.
